I hereby give DANCE FACTORY STUDIOS permission to take and use any still or moving image, being video footage, photographs and/or audio footage depicting my child.
That said still or moving images may be used for any of the following - Advertisements, marketing, leaflets, programmes and any other publicity purposes including on DANCE FACTORY STUDIOS website and online.
The above consent will apply indefinitely unless I/we withdraw our permission in writing, such communication to be sent to THE PRINCIPAL OF DANCE FATORY STUDIOS.
